Colorado Marriage and Family Therapist
Monthly Clinical Hour Log
This excel sheet is to log your clinical hours for Marriage and Family Therapy License. Row and Colum for Total hours have formula embedded already, so you just need to plug in relational, non-relational, case management, and supervision hours. The form should calculate the total for you. Although Colorado DORA does not provide a standard form to keep track of your Marriage and Family Therapy Intern Hours, it is important to do so in organized manner.
Clinical Case Log
This spread sheet is originally in Frank Thomas' article "Weekly Risk/Goal Chart Example" in his book, "Solution Focused Supervision: A Resource-Oriented Approach to Develop Clinical Experties". It is modified from his original chart. You may find this log useful in keeping your case load organized and easily communicating with your supervisor. The spread sheet has sections to enter the rating of client's progress and the client's level of crisis. It will be best printed in Landscape.
Supervision Contract
This is the supervision contract form I use to establish a supervision relationship. This form is useful when your supervisor is outside of the agency you are working at currently because it has a section explaining the ultimate clinical responsibility and the information of the agency supervisor. As we discuss your current progress and your clinical goals, we customize the goal section and the expectation section to fit your needs.
